Frequently Asked Questions
How do I schedule an appointment?
There are several ways you can schedule an appointment. Log into your patient portal and request an appointment, email us at appointments@urologyhsv.com, or call us at (256) 882-3605.
How do I get to your office?
Our address is:
4704 Whitesburg Dr. Ste 100
Huntsville, AL 35802
What happens when I arrive?
Upon arrival to the office, you will check in with our front desk and they will verify your name and date of birth. Please let us know if you have an insurance change or address change.
Patients will be required to fill out yearly paperwork. Please update this to the best of your ability even if things have not changed. You will be required to fill this out every year. We ask that you update medication lists with the front staff at any visit through the year.
New patients or patients that have not been seen in over 3 years will be required to fill out a New Patient Packet. This includes detailed medical and surgical history, consents and pertinent information for the providers.
Once you finish your paperwork you will turn it into the front desk and they will present you with a lid to leave a urine specimen in one of the 3 available restrooms.
Why is it required to leave a urine specimen?
At Urology Specialists we test your urine to see if you have any abnormalities that may effect your plan of care. Some of the procedures and blood draws that we perform can be altered by a urinary tract infection, or certain abnormalities in your urine. If you are being seen to be evaluated for a UTI or UTI symptoms, it is an absolute must that you provide a sample. It is at the individual providers discretion if you do not need one, so please attempt to be prepared every single visit to leave one.
I am receiving text messages, what are these?
Urology Specialists uses a scheduling reminder program to send reminders about your appointment via text and call. It is very beneficial to respond appropriately if you will be at your appointment or need to cancel. Please make sure that your phone number on file is correct so you can participate in our automated system.
Can I fill out my paperwork ahead of time?
Urology Specialists has partnered with Luma Health to not only provide you with appointment reminders but they also are in charge of our intake forms, also known as paperwork. They will send you a text message “checklist” that allows you to fill out your appropriate paperwork. If you have not been seen in 3 years, you will receive our new patient paperwork. If you have been seen in less than 3 years or are being seen for a yearly appointment, you will be prompted to fill out our yearly paperwork.
Why am I having to fill out paperwork again online?
As of Wednesday August 20, 2025 we adopted a new intake system. Once you fill out this paperwork there is a filter in place that it will NOT send you paperwork for 365 days. If you were seen prior to this and filled out paper paperwork in office, our electronic system does not know that. Please attempt to fill out your paperwork online so it can now keep track of it. The paperwork online takes 5-7 minutes and helps our office expedite the check in process.
We do understand that some people do not work technology well and we do have paperwork in office. You will still be sent reminders and checklists for paperwork on your phone.
How do I find out my copay?
For the best results please contact your insurance holder and make sure you ask for the copay to see a specialist. It is the patient’s responsibility to know their copays and deductibles. It is also your responsibility to know what your plan covers before visiting the office.
You are expected to pay co-pays and outstanding balances at check in, PRIOR to being seen by a provider.
What insurances do you NOT accept?
- Medicaid
- Cigna Health Springs
Other insurances may require referrals, or approval to be scheduled. Some insurances may require you to pay upfront. Please contact your insurance providers.
My doctor mentioned a referral/imaging, what do I need to do?
Upon finishing with the provider, they have written instructions for our check out and scheduling department. Some imaging may not be emergent, and they will ask for your phone number and will call you to schedule the appointment. For the emergent scheduling you may be at check out a bit longer while they schedule. Referrals are also handled by our check out, this normally includes filling out a form, gathering the most recent office visit from the clinic and faxing it to the facility. In most cases the facility that we send it to will contact you. This is not a quick process, so we ask for your patience.
Why can I not schedule an appointment out more than 3-6 months?
Our doctors see patients for half a day in office, and then work a surgical schedule whenever they aren’t in the office. They are also required to be “on call” at Huntsville Hospital and Crestwood Medical Center. This schedule changes from week to week and even day to day. The schedule is put out dynamically every 3-6 months, and could have some changes as issues arise. We offer you a reminder card at check out when to schedule your next appointment. You can also email appointments@urologyhsv.com, log onto your patient portal and request an appointment, or call (256) 882-3605.
Why are you not calling me with my results or mailing me a letter?
Your Urology Specialists providers believe, and attempt to provide compassionate “patient first” care to each individual. As part of this approach, the providers will always contact you if there is an abnormal lab and or imaging result. We encourage every patient to review your medical records on our patient portal. We will not contact you if your results are with in a normal range, unless a follow-up is requested by your provider. Please wait a week before calling the office after receiving the results due to giving your provider time to review them.
Why can I not be seen immediately if I call and say I need an appointment?
Urology Specialists is proud to provide you with a myriad of care options and appointment availability. Our providers work half days in office, and are in surgery the other half. There are days that we may refer you to your PCP or Urgent care for a possible urinary tract infection, a minor issue, or we may push a yearly appointment out a couple of weeks. We are grateful you have chosen us for your care, so please be patient when scheduling.
Why am I on hold so long on the phone?
Urology Specialists recently underwent a study with our phone line systems and it shows that our office takes anywhere from 3,000 to 4,000 phone calls Monday-Friday. To help alleviate this burden on the phones, we ask that you use your patient portal to ask questions to your physician’s staff, schedule appointments, or even look at your results. These are sent immediately to the correct personnel and our goal is to contact you within 24-48 hours. For appointment scheduling, you can email us at appointments@urologyhsv.com. If you are experiencing a medical emergency please proceed to your nearest emergency room.
Why can I not get a medication refill when I haven’t been seen in 2 years?
The providers at Urology Specialists require you to be seen every 365 days if you hold a long-term prescription. If you are overdue for a yearly visit please schedule an appointment so we can see you and send in refills. We will not send in prescriptions if you have not been seen.
Note: We will not schedule an emergent appointment due to an over due yearly. It is the patient’s responsibility to know their refills and when they run out.
Why can I not get my testosterone prescription refilled?
You are required to be seen every 6 months in office and obtain a refill prescription as this is considered a controlled substance. No exceptions. Please schedule your appointment ahead of running out.
Why is it taking so long for a Prior Authorization and why are you charging?
Why am I being asked to pay for medical leave forms and my medical records?
Urology Specialists reserves the right to charge for forms and records. You will be notified if you will have to pay.
Why can my issues not be handled in a portal message?
The portal is used to initiate a concern or question. If the provider wants you to be seen in office rather than writing back and forth that is at their discretion. Please refrain from sending multiple messages updating symptoms daily.
Why do I have to pay for PRO-NOX nitrous oxide?
PRO-NOX or nitrous oxide is an elective option for in office procedures such a biopsies and vasectomies. We do not file it with your insurance and it is a $100 payment by the patient. It is not to be included in payment plans; it is an upfront charge, paid in full before receiving the product.
