What is Cystectomy?

When is Bladder Removal Recommended?

A bladder removal surgery is most often recommended in the case of cancer. This is typically when the cancer has progressed beyond the initial stages and has not responded to other treatments. The cancer may begin or spread to the bladder.

You may also need to remove the bladder if you have issues with painful bladder syndrome. This inflammation may cause bladder pain, pelvic pain, pressure, and frequent need to urinate. If you are born with severe abnormalities you may also need to remove the bladder for health and well being.

Types of Bladder Removal

The type of bladder removal surgery and reconstruction you and your urologist decide is for your unique situation. The type of cystectomy may be decided based on your preferences, your reasons for surgery, and your desires after the surgery.

The two main types of bladder removal surgery include:

  1. Partial Cystectomy – removing only a portion of the bladder. This is done when the cancerous or problematic growth is confined to a specific area of the bladder.
  2. Radical Cystectomy- the most common type of cystectomy performed for bladder cancer treatment. A radical cystectomy involves the complete removal of the bladder, nearby lymph nodes, and sometimes other nearby organs. These organs may also include uterus, ovaries, fallopian tubes in women. In men, this may include the removal of the prostate.

What to Expect

As you visit with your urologist, you will discuss the risks and potential outcomes for surgery.

To prepare for the procedure:

  • Discuss with your doctor your desired outcome
  • Decide on a plan for your specific surgery
  • Undergo tests to asses your health and condition
  • Receive general anesthesia for comfort and safety throughout the surgery
  • Undergo surgery whether through traditional, minimally-invasive, robotic-assisted or other techniques
  • Your surgeon will remove the bladder
  • The surgeon will create a new way for urine to exit the body

After Bladder Removal

After the bladder removal, the surgeon will create a new way for urine to exit the body.
This might involve:
  • Constructing a neobladder (a new bladder using a portion of the intestines)
  • Creating a urinary diversion to an external pouch (ileal conduit)
  • Or attaching a segment of the intestine to the skin (ileostomy or colostomy)

This is called urinary diversion. Surgeons are skilled to create an alternative solution for you to hold your urine. If they are constructing a neobladder, they can also attach the tubes that carry urine down from the kidneys. Recovery after the surgery may be challenging, but surgical techniques and advancements have greatly improved.

As you move forward without your bladder, you will likely stay in the hospital for a period of time to manage the pain and discomfort. Your urologist will discuss your diet and activity as you adjust to life without a bladder or if you have had a neobladder procedure.

It is important to attend any follow-up appointments to monitor your recovery and answer any questions you may have. There are some potential risks such as bleeding, infection, blood clots, or organ damage. It is a complex surgery that requires special attention.
